आपके प्रतिष्ठत स्कूल/कालेज में मेरा बेटा/बेटी कक्षा (कक्षा और सेक्शन अलग से लिखें) में पढ़ रहे हैं। कोविड 19 के कारण अभी तक मेरा बेटा/बेटी घर से आन लाइन क्लास अटेंड कर रहे थे। अब जबकि कोरोना के केस कम हो गए हैं और स्कूल/कालेज दोबारा शुरू हो रहे हैं तो मैं चाहता/चाहती हूं कि मेरे बेटा/बेटी आफ लाइन क्लासें स्कूल/कालेज से शुरू करें। इसके लिए मैं अपना कंसेंट देता/देती हूं।
इस दौरान स्कूल/कालेज के कोविड नियमों का मेरे बेटा/बेटी पालन करेंगे। इस दौरान अगर उन्हें बुखार या जुकाम होता है तो मैं उन्हें स्कूल/कालेज नहीं भेजूंगा/भेजूंगी। इसके साथ ही मैं इस बारे में स्कूल/कालेज को भी सूचित करूंगा/करूंगी।
